- Submit Health Office Medication Administration Consent Form – Parent fills out.
- Submit Health Office Physician Medication Order Form – Medical provider fills out.
- A parent, guardian, or responsible adult whom they designate must deliver the medication to the Health Office. They can expect to:
- Fill out the Delivery Log.

- The medication must be in a pharmacy or manufacturer-labeled container. Your pharmacist should provide separate bottles for prescription medications for school and home.

What medication can my student carry with them?
- Any student who uses an inhaler for asthma, insulin for diabetes, or an Epi-Pen for allergic reactions is encouraged to have a supply of this medication for school and home.
